Each therapist at KEBT keeps their own schedule and will be in contact with you about making an appointment. Before your appointment please fill out and return either by *email or *fax (888-889-7312) the following forms found below-
if you are having issues with filling out the form electronically it is okay to fill out manually and send us a photo of the form.
*client information sheet
*signature page of the consent to treatment (please confirm that you have read HIPAA info and initial)
*copy of your insurance card front and back along with state ID or license (you can either scan these or take a photo)
*signed telehealth consent form

In general, we are using the HIPAA compliant platforms, DOXY or Google Meets- we will send you an invite to the session. Please make sure that you have a good working internet connection and have a private place to meet. In general, computers work better than phones- your connection will be more efficient by closing other applications when meeting.
Our building shares a parking lot with the new Gentle Dentistry which faces W. Main. The easiest way to access our lot is to enter through the Gentle Dentistry driveway. We are behind their building. There are multiple parking spaces adjacent to the building.
Each therapist handles their own scheduling and we do not use a receptionist at this time. Please wait in our lobby and your therapist will escort you to their office. In general, we do not ask clients to arrive early for appointments. If your therapist has not arrived to open the building and it is more than 5 minutes past your appointment time, please contact them directly.
To keep our space secure, the door between the lobby and the main area is kept locked. Restrooms are located in the secure area, but we will make every effort to offer access to those waiting. If you have medical needs that require immediate access to a restroom this may be something to discuss with your therapist before the first session.
